    Guide to Community Resources for Seniors and Older Adults – Durham Region

    This guide was produced by the Region of Durham with support from the Government of Ontario. It aims to increase social connection and community engagement among seniors and older adults by increasing awareness of culturally responsive programs and services.  Access the online version of this guide at the Region of Durham’s Age-Friendly Web page.


    Remote Care Monitoring Program:

    Toronto Grace Health Centre’s (TGHC) Remote Care Monitoring (RCM) program utilizes technological advancements to provide comprehensive virtual care encompassing both medical and non-medical needs. Over the past 3 years, RCM has extended its reach to 8,000+ homes across Ontario, exemplifying its spread and scalability.

    In close collaboration with Ontario Health, Home and Community Care Support Services, GRTHealth, and community caregivers, TGHC presents a seamless integrated transitional care model.  RCM clients can access various technologies, including:

    • SOS/Falls/GPS Locating Pendant
    • Passive Home Monitoring System
    • Automated Medication Dispensing Device
    • Medical monitoring devices for chronic disease management
    Eligibility Criteria
    • 60+ years of age
    • Valid OHIP or IFH
    • Stable housing
    • ALC or in the community and at risk of hospital and/or LTC admission
    • Meet the criteria for at least one of the following:
      • Recurrent falls within the last 3 months
      • Acute functional decline (loss of one or more ADLs/IADLs) within the last 3 months
      • Wandering history
      • Forgetfulness with medication adherence
    • Referral must be submitted by a healthcare professional or social service organization.
    • Have an escalation point while in the community (e.g., caseworker, HCCSS coordinator, family member, friend, etc.), whereby the RCM team can communicate non-urgent concerns.

    The Ontario Caregiver Organization (OCO)

    The OCO supports caregivers by being their one point of access to information, so they have what they need to be successful in their role.  The OCO provides supports and tools for caregivers.  Contact the OCO at 1-833-416-2273 or use their Helpline Chat found on their website.
