Report a Workplace Accident, Injury, Hazard or Near Miss

If you require medical assistance, contact the Campus Emergency Response Team (CERT) via Security by phoning 905.721.3211 or ext. 2400 from any campus phone.

All workplace accidents, injuries, hazards, and near misses, regardless of severity, must be reported using the below forms.  This ensures support can be provided and future occurrences can be prevented.

How to Respond

  • What an EMPLOYEE should do

    If you are an employee who has been in an accident or injured while on campus, follow these steps:

    Step 1: Seek first aid or medical emergency assistance as needed

    If you require medical assistance, contact Campus Security, by phoning 905.721.3211 or ext. 2400 from any campus phone, who will dispatch the Campus Emergency Response Team (CERT) and the appropriate emergency responders to your location.

    Step 2: Notify your supervisor immediately

    Please contact your supervisor immediately, or as soon as you are able.

    Even if the injury does not require medical attention or you have not missed work, your supervisor must be notified.

    If your injury worsens, and you later require medical attention, notify your supervisor immediately.

    If you believe you have a work-related illness, a medical condition or disease caused, contributed to, or made worse by your work environment or job duties, this must also be reported to your supervisor.

    Step 3: Complete the Accident Injury Form

    Submit the completed Accident Injury Form within 24 hours.

    If your injury requires health care or time off work

    Your supervisor will be required to work with the Environmental Health and Safety Officer to complete the mandatory Workplace Safety and Insurance Board (WSIB) Employer's Report of Injury (Form 7) within 3-business days of the occurrence.

  • What a SUPERVISOR should do

    If you are a supervisor of an employee who has been in an accident or injured, follow these steps:

    Step 1: Ensure first aid or medical assistance is provided

    If the injured person requires medical assistance, contact Campus Security, by phoning 905.721.3211 or ext. 2400 from any campus phone, who will dispatch the Campus Emergency Response Team (CERT) and the appropriate emergency responders to your location.

    Step 2: Make the Situation Safe

    As needed, Supervisors should respond to workplace injuries as soon as reasonably possible.

    If there is a critical injury, do not disturb the scene. Follow these steps for Critical Injury Reporting.

    Step 3: Ensure Accident Injury Form Completion

    Ensure the injured employee completes the Accident Injury Form within 24 hours.  If the employee is unable to complete the form, you may submit on their behalf.

    Should a previously reported injury escalate, where the employee obtains health care, the Environmental Health and Safety Officer must be notified immediately at 905.261.7455 or marrium.khalid@ontariotechu.ca.

    Step 4: Investigate

    Promptly investigate the accident/injury, using the Accident Injury Investigation Report Form, to determine the cause and take appropriate corrective action to prevent a recurrence. 

    If the injury requires medical care or time off work

    You will be required to work with the Environmental Health and Safety Officer to complete the mandatory Workplace Safety and Insurance Board (WSIB) Employer's Report of Injury (Form 7) within 3-business days of the occurrence.

Reporting Requirements

  • Critical Injury Reporting

    Critical injuries are defined under the Occupational Health and Safety Act (OHSA) as injuries that:

    • place life in jeopardy,
    • produces unconsciousness,
    • results in substantial loss of blood,
    • involes the fracture of a leg or arm but not a finger or toe,
    • involves the amputation of a leg, arm, hand or foot, or but not a finger or toe,
    • consists of burns to a major portion of the body, or
    • causes the loss of sight in an eye

    If a workplace injury meets the definition of critical injury, take the following steps:

    Step 1: Ensure first aid or medical assistance is provided

    Call 911 and contact Campus Security, by phoning 905.721.3211 or ext. 2400 from any campus phone, who will dispatch the Campus Emergency Response Team (CERT) and the appropriate emergency responders to your location.

    Step 2: Secure the Scene

    The scene must remain undisturbed until a Ministry Inspector has arrived, unless saving a life, relieving suffering or preventing major damage.

    The supervisor should go to the scene as soon as possible.

    Step 3: Notify the appropriate people

    Notify the Environmental Health and Safety Officer by phoning 905.261.7455 who will contact the Ministry of Labour, Immigration, Training and Skills Development.

    Step 4: Accident Injury Form Completion

    The injured employee completes the Accident Injury Form within 24 hours.  If the employee is unable to complete the form, their supervisor or dean may submit on their behalf.

    Step 5: Investigate

    The supervisor will promptly investigate the accident/injury, using the Accident Injury Investigation Report Form, to determine the cause and take appropriate corrective action to prevent a recurrence. 

  • Workplace Safety and Insurance Board (WSIB) Reporting

    Injuries that meet the below criteria must be reported to the WSIB within 3-business days of occurrence.  It is imperpative that the Accident Injury Form be completed within 24 hours of the occurrence to ensure there is no delay in reporting.

    Reporting Criteria:

    • Employee requires health care*
      • *Health care is considered services requiring the professional skills of a health care practitioner (e.g., doctor, nurse) beyond first-aid that does not require the professional skills of the practitioner (e.g., a coworker puts a band-aid on a minor cut), and sevices provided at hospitals and health facilities.
    • Is absent from regular work
    • Earns less than regular pay for regular work (e.g., part-time hours)
    • Requires modified work at less than regular pay, or
    • Requires modified work at regular pay for more than seven calendar days following the date of accident/injury.

    Note: Should a previously reported injury escalate to the above criteria, the Environmental Health and Safety Officer must be notified immediately at 905.261.7455 or marrium.khalid@ontariotechu.ca.
