Report a Workplace Accident, Injury, Hazard or Near Miss

If you require medical assistance, contact the Campus Emergency Response Team (CERT) via Security by phoning 905.721.3211 or ext. 2400 from any campus phone.

All workplace accidents, injuries, hazards, and near misses, regardless of severity, must be reported using our Accident Injury and Hazard/Near Miss Reporting Forms within 24-hours of occurrence and according to this Accident Injury Management document.
